The Family Practice of Holyoke is a Medicare-certified rural health center located in the west wing of Melissa Memorial Hospital. Our clinic staff is very dedicated to providing you with the highest quality healthcare services. You can take comfort in knowing that we will provide you and your family with a full range of experienced, compassionate healthcare services close to home. We encourage you to make suggestions aimed at improving our practice.

7:00 AM - 5:00 PM* Monday - Friday
*final appt is at 4:30 PM
Please inform our receptionist of the reason for your visit so that enough time can be scheduled. If you cannot keep your appointment, please call and cancel in advance. Another patient may then use this time slot. Please plan to arrive 15 minutes before your appointment.
Prescriptions
Prescriptions and refill requests can be sent to our convenient, on-site pharmacy or any pharmacy of your choosing. If you have questions about your refills, please contact the clinic during office hours. Please allow 72 hours for off-site refills. Please note that the on-call and weekend doctor cannot refill your prescriptions.

Your medical records are confidential and, as such, will not be shared or released without your written consent.
