What is Chronic Care Management?

If you have two or more serious chronic conditions such as diabetes and arthritis that you expect to last for at least a year, Medicare may pay for a healthcare provider's assistance in managing your care for those conditions. You will have to pay a monthly fee, and the Part B deductible and coinsurance will apply. However, if you have supplemental insurance, including Medicaid, it may help cover the monthly fee.

How it works?

Chronic care management includes a comprehensive care plan that lists your health problems, goals, medications, other providers, community services you have and need, and other information about your health. It also explains the care you need and how your providers will coordinate it. If you agree to get this service, your provider will ask you to sign an agreement for you to receive this set of services monthly.

CCM Team

Once you agree to this service, your provider will prepare the care plan for you or your caregiver, aid you with medication management, review your medicines and how you take them, and help you with other chronic care needs. You will also have a care team that will have monthly check-ins to see how you are doing, note any changes or symptoms you may be noticing and remind you of any appointments or follow up information.
