Update on U.S. Department of Transportation

As of January 2018 the U.S. Department of Transportation is changing their custody and control forms for drug test collection. You can visit the website www.transportation.gov/  for a link to the changes. You can continue to use the old form and deplete your current supply, but after June 1, 2018, we will no longer accept previous forms and you will need to contact your supplier for the newest, updated forms.

In addition, Melissa Memorial Hospital is requesting all DERs to fill out an authorization form if your participant is presenting without the notice of selection. The authorization form enclosed should be presented with the employee participating in the drug test collection at the time of service, or faxed directly to the MMH laboratory (970-854-3834). The authorization form is to ensure we best serve your needs with accurate testing and billing information.

We are currently updating our files and want to ensure we have your correct Designated Employee Representative (DER) and contact information. Please verify your mailing, billing address and phone number and fax listed above, and inform the laboratory staff of any changes for accuracy.

As a reminder, our hours of collection are Monday through Friday 8am to 4pm. These are done by appointment only, please check in at admissions in the main lobby entrance. Please feel free to contact lab manager, Dan Lakandili, at (970) 854 2241 x707 with any questions or concerns.
